Case coordination related to SAP requirements
Review of completion documentation
Follow-Up SAP Evaluation
Required SAP reports
FMCSA Clearinghouse reporting when applicable
Follow-up testing plan
Guidance throughout the Return-to-Duty process

Initial SAP Evaluation — $225

Includes the initial evaluation and development of the required education and/or treatment recommendation.
Follow-Up SAP Evaluation — $200
Follow-Up SAP Evaluation — $200
Completed after the employee has finished the SAP's required education and/or treatment recommendation.
Additional Follow-Up Evaluation — $125
Additional Follow-Up Evaluation — $125
If additional follow-up evaluation is required because the employee has not yet demonstrated satisfactory compliance, an additional evaluation may be necessary.

Employees who do not purchase the Complete DOT SAP Package may pay for services separately:
$225 Initial Evaluation + $200 Follow-Up Evaluation = $425 total
Choosing the $400 Complete DOT SAP Package saves $25 when the full SAP process is required.

Important Information
Education and treatment services are not included in SAP fees. Any costs charged by an outside education or treatment provider are paid separately to that provider.
Return-to-Duty and follow-up drug/alcohol testing costs are also separate from SAP service fees unless otherwise arranged by an employer or testing provider.
Payment for SAP services does not guarantee a particular recommendation, determination of compliance, Return-to-Duty test result, or employment outcome. SAP determinations are made in accordance with applicable DOT requirements.
